What if your health plan pays one price for a prescription, but the pharmacy receives something completely different?

That is known as spread pricing.

A PBM may charge the health plan one amount, reimburse the pharmacy a lower amount, and keep the difference.

In 2018, Ohio examined both sides of the transaction in its Medicaid program.

The findings were significant:

โœ” Nearly 9% spread across all drugs
โœ” About 31% on generics
โœ” Almost $225 million in one year

And the only reason those numbers became visible was because the state decided to look.

The FTC has also estimated that the three largest PBMs collected around $1.4 billion through spread pricing on specialty generic drugs from 2017 through 2022.

So here is the question:

If employers and health plans cannot see both numbers, how are they supposed to know where their prescription dollars are going?

Patients deserve transparency. Pharmacies deserve transparency. Employers and taxpayers deserve it too.

Open enrollment usually focuses on premiums.

But there is another question worth asking:

Does your plan pass prescription drug rebates through to you at the pharmacy counter?

Many people assume negotiated rebates automatically mean lower prescription costs.

That is not always the case.

Depending on how a plan is structured, the savings may stay within the system instead of directly reducing what the patient pays.

That is why transparency matters.

Employers and health plans can ask:

โœ” Where do the rebates go?
โœ” Are savings passed through at checkout?
โœ” Are patients paying based on list price or net cost?
โœ” How transparent is the PBM agreement?

There are also signs that the traditional high-price, high-rebate model is starting to change, especially in areas like insulin pricing.

The bigger question is whether those changes will reach patients consistently.

What happens when the company deciding how much pharmacies get paid also owns the pharmacy getting paid?

That is one of the concerns raised in a January 2025 FTC report on specialty generic drugs.

The FTC found that the three largest PBMs marked up many specialty generics dispensed through their own affiliated pharmacies by hundreds, and sometimes thousands, of percent.

Between 2017 and 2022, those pharmacies generated more than **$7.3 billion above what the drugs cost them to acquire**.

The report also found that PBMs reimbursed their own affiliated pharmacies at higher rates than independent pharmacies for nearly every specialty generic reviewed.

By 2023, affiliated pharmacies controlled **68% of specialty drug dispensing revenue**.

Why does this matter?

Because independent pharmacies are expected to compete in a system where the same companies may influence pricing, reimbursement, and pharmacy ownership.

That raises a very simple question:

Is that a fair marketplace?
