Did you know your pharmacy usually doesn't get paid directly by your health insurance company?

When you visit a doctor's office, the payment process generally goes through your health insurer.

But when you pick up a prescription, there is often another company involved.

It is called a Pharmacy Benefit Manager, or PBM.

PBMs work between insurance companies, drug manufacturers, and pharmacies to help manage prescription drug benefits.

Some of the largest PBMs include CVS Caremark, Express Scripts, and OptumRx.

And they have more influence over your prescriptions than you might realize.

PBMs can help determine:

✔ Which medications your insurance covers
✔ How much your pharmacy gets reimbursed
✔ Which pharmacies are included in your network
✔ How prescription claims are processed
✔ What you pay when picking up your medication

Here is why that matters.

Your local pharmacy may fill your prescription, provide counseling, answer your questions, and help manage your medications.

But the amount it receives for that prescription is often determined through a PBM-managed payment system.

Why does your pharmacy get paid differently than your doctor's office?
It is a question most patients have probably never considered.

When you visit a doctor's office or hospital, payments are generally handled through your health insurance company's medical claims system.

But when you fill a prescription, another company often enters the picture: a Pharmacy Benefit Manager (PBM).

Should a company responsible for lowering prescription drug costs make more money when those costs go up?

More patients. More prescriptions. More people trusting their local pharmacy with their care.

And yet this pharmacy owner describes sitting alone after closing, looking at the bank account and wondering whether there would be enough money to pay the upcoming drug bill.

That moment says so much about what independent pharmacies are facing.

A pharmacy can be busy and growing while still struggling financially.
